Description

Job Opportunity: RN Medical/Surgical

Location: Nashville, Tennessee
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Start Date: January 5 (Non-negotiable)
Shift: 36 or 48 hours per week


Key Requirements & Credentials

Licenses & Certifications

  • Nursys Verification: Must be completed within 30 days prior to start. Disciplinary actions require additional documentation.
  • Current Nursing License: Primary source verification within 30 days prior to start.
  • BLS (AHA): Required prior to onboarding.
  • NIH Certification: Group A updated yearly; Groups B-F updated every 2 years.
  • Driver’s License/State ID: Must be current and match address on file.

Testing & Exams (Required Before or At Onboarding)

  • Ascension Medical Surgical Exam: Minimum 80% score, max 2 attempts, updated every 3 years.
  • Ascension EKG Exam: Minimum 80% score, max 2 attempts, updated as per latest edition.
  • Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics; minimum 80% score, updated yearly.
  • Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation.
  • Ascension Skills Checklist: Clinical skills checklist, updated annually.

Health & Immunization Requirements

  • MMR: Proof of two vaccines or positive IGG titer; no declinations accepted.
  • Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider; no declinations accepted.
  • Tdap: Valid within 10 years; no declinations accepted.
  • Hepatitis B: Documentation of vaccination series completion or signed declination.
  • Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); declinations accepted with mask requirement.
  • 2 Step PPD or IGRA: Recent testing required with annual TB questionnaire. Positive TB requires chest X-ray within 1 year.
  • Fit Test: Required for all travelers; preferred before start, can be done onsite.
  • Health Statement: Statement of good health or physical exam prior to first assignment, updated annually.
  • 12 Panel Drug Screen: Within 30 days prior to start, updated annually.

Onboarding & Documentation

  • Ascension Resume/Profile: Required for submission.
  • Professional References: At least two professional references required before first assignment.
  • Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; updated annually.
  • Employment Verification: Past 7 years, updated annually.
  • Education Verification: Relevant to clinical work, completed within 90 days of start.
  • Ascension Badge Photo: Forward-facing headshot with white or blue background, no filters.
  • Ascension Onboarding Requirements Education Attestation: Completion of pre-hire modules and attestations.
  • Ascension Nursing Contract Worker Onboarding: Completed onsite.
  • Ascension Nova StatStrip: Pre-hire modules for non-allied positions.
  • Ascension Confirmation of Assignment: Provided upon initial assignment and updates for extensions.
  • Ascension EPLS: Must be run within 90 days of start and rerun if break in service > 90 days.

Optional

  • Ascension Unit Specific Checklist: Optional for onboarding; uploaded by unit leader within a week.
  • Positive PPD - Negative Chest X-Ray and Annual TB Questionnaire: Optional but recommended for those with positive TB history.
  • Hiring Manager Evaluation: Post-assignment evaluation.
  • Ascension Name Change Document: Legal documentation if applicable.

This position requires strict adherence to credentialing, testing, and health requirements prior to and during the assignment. The start date is fixed, and the contract length is approximately 13 weeks.
