Description
Job Opportunity: CT Technologist
Location: Plymouth, Indiana
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Work Schedule
- Sunday: 6:30 PM – 7:00 AM
- Wednesday: 10:30 AM – 11:00 PM
- Friday: 10:30 AM – 11:00 PM
- Weekend call coverage every 4th weekend
Required Credentials & Certifications
- ARRT Certification (CT)
- BLS Certification (American Heart Association)
- License/Certificate Verification (must be dated within 30 days of start)
- Fit Test
- Annual TB Questionnaire
- Trinity 10 Panel Drug Screen (completed within 30 days of start)
- Trinity Code of Conduct Form
- Trinity I9 Form
- Trinity Confidentiality & Security Agreement
- Trinity Integrity and Audit Services Form
- Trinity Joint Commission Testing (CORE Exam within the last year)
- Trinity Color Vision Test
- Trinity Latex Allergy Questionnaire
- COVID-19 Vaccine (proof of booster, declination or exemption required)
- MMR Vaccination or IgG titer proof
- Hepatitis B Vaccination series (proof required)
- Influenza Vaccine (proof, declination or exemption required)
- 2 Step PPD OR IGRA (Quantiferon/Tspot)
- Varicella Vaccination or IgG titer proof
- Physical/Health Statement (exam within last year)
- Tdap Vaccine (current, non-expired; declination accepted if no pediatric interaction)

- Trinity Conflict of Interest Disclosure Form
- Criminal Background Check (Federal, Nationwide, County - last 7 years, including SSN verification)
- Education Verification (highest level of academic education)
- EPLS (Excluded Parties List System) Check

- NSO (National Sex Offender) Check
- Trinity System Access Form (must be completed and uploaded one week before start)
- Trinity Skills Checklist (clinical contractors only)
- Trinity OFAC Sanctions Search (within 30 days of start)
