Job Details

• Shift: 5 days a week, 8 hours per day (8:00 AM - 5:00 PM)
• Area of Coverage: San Jose/South Bay/Santa Clara County
• Specialty: Therapy/Rehabilitation (Physical Therapy)

Required Qualifications

• Valid California Physical Therapist (PT) License
• Basic Life Support (BLS) certification (AHA or ARC)
• Home Health experience required

Background and Compliance Requirements

• 7-year background check within 6 months of start, including:

  • Social Security trace with all aliases
  • County searches for last 7 years
  • Nationwide sex offender search
  • Education verification (highest level achieved)
  • FACIS Level 3 screening within 2 weeks of start and monthly thereafter
  • Motor Vehicle Report (required for all candidates)
  • EPLS/GSA/SAM and HHS/OIG checks (updated annually)
• Drug screening: Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids due to CA law)
• Employment application with full 7 years of work history, no gaps
• Professional references as required by agency (not client)

Certifications and Competencies

• BLS (Basic Life Support) - AHA or ARC only
• Other certifications (e.g., ACLS, PALS) as applicable by position but not specifically required for this role
• Medication/Pharmacology competency exams as applicable

Health and Immunization Requirements

• Physical exam within 1 year of start (must indicate cleared for work)
• Fit mask test within 1 year, renewed annually (exceptions apply)
• Immunizations and titers:

  • Tetanus/Diphtheria/Tdap (within 10 years)
  • Negative TB test or chest x-ray within 90 days, renewed annually
  • Measles (Rubeola), Mumps, Rubella: Proof of immunity or vaccination
  • Hepatitis B series or signed declination
  • Varicella immunity or vaccination
  • Seasonal flu vaccination during flu season (Nov 1 - Apr 30) or signed declination
• Drug testing: Negative 9-panel drug screen within 6 months of start

• Personal or auto insurance required for home health/driving positions, effective for assignment duration
