Job Opportunity: RN - Case Management (Travel)

Location: Dover, New Jersey

Job Description

• Coordinate management of care to ensure optimal resource utilization, service delivery, and compliance with external review agencies.
• Provide ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and evaluation of individual patient needs.
• Enhance quality of patient management and satisfaction to promote continuity of care and cost-effectiveness.
• Integrate functions of case management, utilization review, management, and discharge planning.
• Serve as a patient advocate addressing holistic patient needs and preferences.

Work Schedule

• Monday to Friday, 8:00 AM to 4:30 PM

Required Qualifications

• Registered Nurse (RN) license valid in New Jersey or multi-state RN license.
• BLS certification through the American Heart Association.
• Minimum of 2 years of RN Case Management experience, specifically in LTAC or STACH settings.

Additional Information

• Prior travel experience is not explicitly required.
• Experience with electronic medical records (EMR) systems is expected but not specified.
