Job Opportunity: PT Home Health (Travel/Contract)

Job Details

• Shift: Monday to Friday, 8:00 AM – 4:30 PM (5x8 days)
• Call: Required 1 Sunday every 5 weeks
• Weekend Rotation: Every 5th week, take 2 days off during the week and work Saturday and Sunday to perform admissions
• Coverage Area: 13 counties in Missouri (including Andrew, Atchison, Buchanan, Clay, Clinton, Daviess, Dekalb, Gentry, Harrison, Holt, Platte, and Worth)
• Dress Code: No printed scrubs; black, grey, or khaki pants with polo or scrub top

Required Qualifications

• License: Valid Missouri Physical Therapist (PT) license (Primary source verification required within 30 days of start)
• Certifications:

  • BLS (Basic Life Support) - AHA or ARC accepted
  • DPT (Doctor of Physical Therapy) degree
• Experience: Minimum 1 year of experience in Home Health Physical Therapy required
• Prior travel experience: Not explicitly required

• Background Checks:

  • National Criminal Check
  • State and County Searches (10 years of all addresses and aliases)
  • SSN Trace with all aliases
  • National Sex Offender Registry
  • EPLS/GSA/SAM
  • HHS/OIG
• Education Verification: Official transcript or diploma with graduation date
• State Specific Search: Missouri EDL Search within 30 days of start
• Health Documents:

  • Physical exam within last 12 months (not required annually)
  • Fit mask testing completed onsite
  • Negative TB test or chest X-ray within 1 year, with annual updates
  • Immunizations or declinations for:

    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
  • Drug screen within 30 days of start (not required annually)
  • Seasonal flu vaccination or approved medical/religious exemption (mask required if exempt)
• References: Two professional references within 3 years of start

• Work History Verification: Verification of most recent employer
