Job Opportunity: Registered Nurse - Home Health

Location: Kennesaw, Georgia
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
  • Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
  • Conduct home visits and periodic office meetings.
  • Average patient load: 5-8 patients per day.
  • Types of cases: Start of Care, Wound Care, Follow-up Visits, etc.
  • Schedule: Monday to Friday, 8 AM to 5 PM.
  • Patient visits include both care facilities and patient homes.
  • Must be willing to travel up to 75 miles from the home clinic.
  • Mileage reimbursed at IRS standard rate (patient home to patient home; commute to/from first and last patient not billable).
  • On-call requirement: Yes, 1-week rotation, 5 PM to 8 AM, every 3-6 weeks.
  • Dress code: Scrubs.
  • Charting systems: Proficiency required in HomeCare HomeBase (HCHB) within last 6 months; RNs also required to be proficient in OASIS.

Requirements

  • Valid Registered Nurse (RN) license in Georgia.
  • Minimum of 1 year of recent Home Health experience within the last year.
  • Hospice experience.
  • BLS certification issued by the American Heart Association or American Red Cross.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
  • Experience working with adult patients.
  • Proficiency with HomeCare HomeBase (HCHB) documentation system.
  • Proficiency with OASIS documentation system.

Productivity Expectations

  • Weekly productivity units: 30 points.
  • Daily patient visits: 5-10 patients, equating to roughly 6-8 points per day.
  • Weekly travel: Approximately 30-40 miles per day.

Technology & Documentation

  • Must be proficient with HomeCare HomeBase (HCHB) and OASIS documentation systems.
  • Use of Outlook for communication.

Compliance & Onboarding Requirements

  • Background checks including 7-year history, SS trace, county, nationwide, sex offender, and state-specific searches.
  • Drug screening: 10-panel required.
  • Health documents: Physical exam within 12 months, negative TB or chest X-ray within 12 months, immunizations/titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella, Flu vaccination or declination, COVID vaccination or declination.
  • Fingerprinting required depending on state placement.
  • Education verification required.
  • References: Two references required.
  • Work history verification: At least two most recent employers or last 7 years.
  • Various facility-specific compliance documents and training must be reviewed and signed prior to start (e.g., HIPAA, Code of Conduct, Compliance Training, Privacy Policy, Confidentiality Forms, Home Health and Hospice Core Policies, COVID-19 Policy, etc.).
  • Personal or auto insurance required for home health placements; policy must be current and under candidate’s name or list candidate as a driver.

Additional Notes

  • Must be willing to adhere to all state-specific requirements and documentation.
  • Some positions may have a Baylor shift schedule (4x10s Friday-Monday), but most are standard 5x8 Monday-Friday.
  • Floating to nearby clinics may be possible if patient census is low to meet financial needs.

This position is ideal for experienced Home Health RNs with recent specialty experience who are comfortable with travel within a 75-mile radius and proficient in required documentation systems.

Cons

Generally speaking, Host Healthcare doesn't always have the highest rate on the market. Having gone with competing agencies in the past, I have often been disappointed and unsupported and ultimately found it wasn't worth the small pay bump
