Job Details

• Outpatient setting only
• Work schedule: 4 shifts per week, each 10 hours long
• Shifts start between 8:00 - 8:30 AM
• Shared housing provided at no cost

Required Qualifications

• Valid Massachusetts Physical Therapist (PT) license
• Basic Life Support (BLS) certification
• Specialty: Physical Therapist Outpatient
• Years of experience: Not explicitly stated; assume minimum 0+ years (no higher requirement indicated)

Required Credentials and Compliance

• Completion of FEMA ICS 100 Training (mandatory, no exceptions)
• License/Certificate verification (primary source verification required within 30 days of start)
• Current BLS certification (AHA preferred)
• Documentation of immunizations and health screenings including:

  • MMR (Measles, Mumps, Rubella) vaccination or immunity proof
  • Influenza vaccine (required during flu season)
  • TB test within 3 months of start date (skin test or blood test)
  • Health statement signed by MD/DO/NP/PA within 1 year
  • Hepatitis B vaccination or immunity (strongly preferred)
  • Tdap vaccine within 10 years (preferred)
• Respirator Fit Test (N95) within 1 year of start date, updated annually
• Criminal background checks including:

  • MGB Criminal Background Check
  • Massachusetts CORI check
  • National Sex Offender Registry check
  • Exclusion checks (OIG, GSA, Medicaid exclusion lists)
• Verification of education and work history (past 7 years)
• Submission of professional references (2 references, including at least 1 supervisor)
• Skills checklist completed within 1 year of submission

Additional Information

• Must comply with all onboarding and credentialing requirements prior to start
• No religious or medical exemptions allowed for flu vaccine or N95 fit testing
• Housing agreement required if staying in provided housing
