Job Opportunity: Outpatient Physical Therapist (Travel/Contract)

Job Details

• Start Dates: May 18 or June 1
• Shift: 4 days per week, 10-hour shifts (two days 7am-5pm, two days 7am-6pm)
• Setting: Outpatient Orthopedic clinic with treatment rooms surrounding a large gym space
• Patient Care: 60-minute one-on-one sessions, no overlapping patients

Required Qualifications

• Valid Physical Therapist license in Wisconsin
• Minimum 1 year of Physical Therapist experience within the last year
• Completion of at least one travel assignment
• Basic Life Support (BLS) certification
• Experience in Rehabilitation, Outpatient, Clinic, and Inpatient settings

Compliance & Screening Requirements

• 7-year background check including SS trace and county searches
• Physical exam within 1 year of start
• Negative TB test (QGold) within 1 year or chest X-ray within 60 days of start
• Drug screening: Negative 10-panel drug screen with expanded opiates within 14 days of start
• Seasonal flu vaccination or approved medical/religious exemption required during flu season (Oct-Mar)
• Professional references: 2 verified within 2 years

• Annual competency exams and skills checklists required
• Photo for badge and confidentiality statement must be completed prior to start
