Required Qualifications
- Current RN license in Indiana with Nursys verification within 30 days prior to start
- BLS certification (American Heart Association)
- Minimum years of experience: Not explicitly stated; assume standard for Medical/Surgical RN (typically 1+ years)
- Pharmacology 1 or 2 exam with minimum 80% score, updated every 3 years
- Medical Surgical exam with minimum 80% score, updated every 3 years

- Immunizations and health requirements:
- MMR: documented proof of two vaccines or positive IGG titer (no declinations accepted)
- Hepatitis B: vaccination series completion or signed declination
- Influenza vaccine (seasonal; exemptions accepted with mask requirement)
- Varicella: documented proof of immunity or two doses of vaccine (no declinations accepted)
- Tdap: valid within 10 years (no declinations accepted)
- TB screening: Positive IGRA or PPD results, negative chest X-ray within 1 year, and annual TB questionnaire
- Health Statement or physical exam prior to assignment, updated annually
- 12 Panel Drug Screen within 30 days prior to start, updated annually
- Criminal background check including Social Security Trace, OIG Search, National Sex Offender, and others within 30 days prior to start, updated annually
- Employment and education verifications within 90 days prior to start
- Skills checklist completed within 1 year of start, updated annually
- Driver’s License/State ID matching home state for compact license (if applicable)
- Professional references: at least two prior to first assignment
- Resume/Profile submission required

Generally speaking, Host Healthcare doesn't always have the highest rate on the market. Having gone with competing agencies in the past, I have often been disappointed and unsupported and ultimately found it wasn't worth the small pay bump
