Job Opportunity: Physical Therapist (Home Health)
Location: Beaver Dam, Wisconsin
Contract Length: 13 weeks
Job Details
- Specialty: Home Health Physical Therapy
- Setting: Home Health
- Typical Caseload: Approximately 5 patients per day (varies by visit/evaluation type)
- Patient Types: Geriatric, Joint Replacement, Stroke, CHF/COPD, Parkinson’s
- Work Hours: 8:00 AM – 4:30 PM, self-scheduling available
- Weekend/Holiday Requirements: None
- Dress Code: Solid color scrubs (any color), closed-toed shoes
Required Certifications & Skills
- BLS Certification (required)
- Experience with OASIS documentation
- Lymphedema certification is a plus
- Rehab aides (PTAs) available and supervised by PT
Orientation & Training
- Computer-based training
- 1:1 shadowing in the field
Notes for Candidates
- This position involves providing physical therapy services in patients’ homes.
- Candidates should be comfortable managing a varied caseload including complex diagnoses such as stroke and Parkinson’s.
- Self-scheduling allows flexibility within the stated work hours.
Required Qualifications
- Valid Physical Therapist license in Wisconsin
- Minimum of 2 years of Physical Therapist experience, including experience within the last year
- BLS Certification
- Experience with OASIS documentation
