Job Opportunity: Physical Therapist (Home Health)

Location: Beaver Dam, Wisconsin
Contract Length: 13 weeks

Job Details

Specialty: Home Health Physical Therapy
Setting: Home Health
Typical Caseload: Approximately 5 patients per day (varies by visit/evaluation type)
Patient Types: Geriatric, Joint Replacement, Stroke, CHF/COPD, Parkinson’s
Work Hours: 8:00 AM – 4:30 PM, self-scheduling available
Weekend/Holiday Requirements: None
Dress Code: Solid color scrubs (any color), closed-toed shoes

Required Certifications & Skills

BLS Certification (required)
Experience with OASIS documentation
Lymphedema certification is a plus
Rehab aides (PTAs) available and supervised by PT

Orientation & Training

Computer-based training
1:1 shadowing in the field

Notes for Candidates

This position involves providing physical therapy services in patients’ homes.
Candidates should be comfortable managing a varied caseload including complex diagnoses such as stroke and Parkinson’s.
Self-scheduling allows flexibility within the stated work hours.

Required Qualifications

Valid Physical Therapist license in Wisconsin
Minimum of 2 years of Physical Therapist experience, including experience within the last year
BLS Certification
Experience with OASIS documentation

Generally speaking, Host Healthcare doesn't always have the highest rate on the market. Having gone with competing agencies in the past, I have often been disappointed and unsupported and ultimately found it wasn't worth the small pay bump
