Location: Mayfield Heights, Ohio

Job Summary

Seeking a Registered Nurse for a Medical-Surgical/Telemetry unit. This is a travel/contract position in Ohio.


Required Qualifications

  • Valid Registered Nurse (RN) license in Ohio.
  • Minimum 1 year of recent clinical experience in Medical-Surgical/Telemetry within the last year.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
  • Current ACLS certification issued by the American Heart Association or American Red Cross.
  • Current BLS certification issued by the American Heart Association or American Red Cross.
  • NIH Stroke Scale (NIHSS) certification.

Certifications Required

  • BLS (Basic Life Support): AHA (Instructor or Provider) or American Red Cross; RQI acceptable. Must be current at start.
  • ACLS (Advanced Cardiac Life Support): Required for Medical-Surgical/Telemetry. Must be current at start.
  • PALS (Pediatric Advanced Life Support): Preferred but not required for this specialty.
  • NRP (Neonatal Resuscitation Program): Not required for MS/Tele.
  • TNCC (Trauma Nursing Core Course): Required only for ER, not for MS/Tele.
  • NIH Stroke Certification: Preferred but not required.
  • State License Verification: Primary source verification within 30 days of start. License must be current and name must match official documents.

Background and Compliance Requirements

  • Government-issued photo ID required (Driver’s License or State ID).
  • Comprehensive background check including FCRA compliant 7-year search, SSN trace, sex offender registry, OFAC, FACIS Level 3, education verification.
  • Monthly EPLS/GSA/SAM and HHS/OIG checks.
  • FBI and BCI fingerprinting required within 12 months of start, updated annually. Fingerprints must be completed under nursing code 4723.09. Facility will also conduct fingerprinting onsite.
  • Physical exam within 1 year of start confirming ability to perform essential job functions. Latex allergy notification required if applicable.
  • Fit mask test (3M N95) within 12 months of start, updated annually. PAPRs not provided for failure due to facial hair unless approved exemption submitted.
  • Immunizations and health screenings required:
    • Tdap vaccine every 10 years (no declinations accepted).
    • Negative TB test or chest X-ray with annual questionnaire if history positive.
    • MMR (Measles, Mumps, Rubella) vaccinations or positive titers with boosters if needed.
    • Hepatitis B vaccination or titer or declination accepted with agency-approved exemption.
    • Varicella vaccination or positive titer with booster if needed.
    • Annual flu vaccination required (no declinations accepted). Medical/religious exemptions must be approved by agency and facility.
  • Drug screening: Standard 10-panel drug screen within 12 months of start, updated annually. Instant drug screens not accepted.
  • Eye exam (Snellen) within 1 year of start.

Orientation and Training

  • Completion of facility-specific online onboarding modules prior to start is mandatory. Modules must be completed on a computer using Google Chrome; cannot be completed on a phone.
  • Baseline competency exams in medication administration and ECG interpretation required for all bedside RNs (except OR RNs). Passing scores: 90% medication, 80% ECG. Remediation and retake opportunities provided if needed.
  • SafeMedicate prep materials provided for exam preparation.

Documentation and History

  • Employment application must include at least 3 years of prior work history with a minimum of 2 years clinical acute care experience in a similar area.
  • Two professional references required from employment within the last year (manager, supervisor, charge nurse, or director). Co-worker references not accepted.
  • Skills checklist required annually.

Additional Notes

  • Candidates with gaps in employment greater than 30-40 days may need to update physical and eye exams.
  • Candidates must carry government-issued photo ID at all times during assignment.
  • Fingerprinting results must be received prior to contract extension. Candidates may start with fingerprinting pending if completed before start date.
  • Facility requires legal name consistency across all documents.
