Description
Job Opportunity: Respiratory Therapist
Location: Mason City, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Work Schedule
- Every third weekend
- Holiday rotation including Christmas
Required Credentials and Certifications
Certifications & Licenses (Must be current and verified within 30 days of start)
- BLS (American Heart Association certification required for all direct patient care personnel)
- ACLS (Required for RNs in critical care areas)
- Valid professional license/certification with online verification
Health & Immunizations
- Fit Test
- COVID-19 Vaccine (including booster or approved exemption)
- Influenza Vaccine (proof or exemption for current flu season)
- MMR (proof of 2 vaccinations or immunity titer)
- Hepatitis B vaccination series (proof of vaccinations required; titers not accepted)
- Varicella (proof of vaccination or immunity titer)
- 2 Step PPD OR IGRA (TB test) with chest x-ray if positive (per regional requirements)
- Tdap vaccine (declinations accepted if no pediatric interaction)
- Physical/Health Statement confirming fitness to work (exam within last year)
- Trinity 10 Panel Drug Screen (completed within 30 days of start, no THC)
Professional Requirements
- Trinity Code of Conduct Form (signed)
- Trinity I9 form (completed by agency)
- Confidentiality and Information System Security Agreement (signed)
- Integrity and Audit Services Form (signed shortly before start)
- Non-Employee Upload Form (pages 1-3 and 28 completed and uploaded)
- Specialty Specific Competency Testing (copy of specialty exam within last year)
- Criminal Background Check (Federal, Nationwide, County for last 7 years, including SSN verification)
- Education Verification (highest academic degree or diploma verified)
- EPLS (Excluded Parties List System) check within 30 days
- GSA/OIG Exclusion check within 30 days
- Joint Commission CORE Exam within last year
- NSO (National Sex Offender) check within 30 days, including state-specific checks for MI and IA
- System Access Form (completed and uploaded one week before start)
- Skills Checklist (provided by agency for clinical contractors)
- Conflict of Interest Disclosure Form (signed)
- OFAC Sanctions Search (completed within 30 days)
- Child and Adult Dependent Abuse Training (required for Iowa assignments)
- Iowa Sing Report (Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check within 30 days prior to start)
