Job Opportunity: Physical Therapist (PT) - Home Health (Travel/Contract)

Job Description

• Provide physical therapy services in patients' homes (home visits).
• Participate in periodic office meetings.
• Average patient load: 5-6 patients per day.
• Types of cases: Start of Care and Follow Ups.
• Schedule: Monday to Friday, 8 AM to 5 PM (most shifts are standard 5x8s; some locations may have 4x10s Friday-Monday).
• On-call: Yes, approximately once a month.
• Travel: Must be willing to travel up to 75 miles from home clinic; daily travel mileage reimbursed at IRS standard rate (patient-to-patient travel compensated, commute to/from first and last patient is not billable).
• EMR systems used: HomeCare HomeBase, Oasis, Outlook.
• Potential for hire at the end of the contract.

Required Qualifications

• License: Valid Physical Therapist license for Georgia (license must be in hand; pending licenses accepted only if they can be obtained within a few days).
• Experience: Minimum 1 full year of recent Adult Home Health experience (recent experience required; first-time home health therapists not accepted).
• Certifications: Current CPR and BLS certification (AHA, National CPR Foundation, or ARC accepted).
• Reliable transportation and a current driver's license.
• Agency-required liability insurance.

Additional Requirements

• Must be willing to discuss weekend requirements and on-call expectations during the interview (varies by location).
• Must be able to meet productivity expectations (approximately 25-30 weekly units, 30 points per week).
• Must complete various background checks, verifications, and compliance documents prior to start, including but not limited to:

  • Standard 7-year background check including SS trace, county, nationwide, sex offender, and state-specific searches.
  • Drug screening (10-panel).
  • Education verification.
  • Physical exam within 12 months confirming good health and no communicable diseases.
  • Negative TB test or chest X-ray within 12 months.
  • Immunization or titer proof for Measles, Mumps, Rubella, Hepatitis B, Varicella.
  • COVID-19 vaccination or declination.
  • Various state-specific and facility-specific compliance forms and attestations.
• Must review and sign multiple compliance, confidentiality, and policy documents prior to start and annually if working more than one year consecutively.

• Locals are accepted at the same rate as travelers.
• Candidates must have reliable transportation and be prepared for home health travel demands.
