Job Opportunity: Registered Nurse - Home Health
Location: Salem, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits with periodic office meetings.
- Average patient load: 5-8 patients per day.
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
- Must be willing to travel up to 75 miles from home clinic; average weekly travel approximately 250 miles.
- Schedule: Monday to Friday, 8 AM to 5 PM.
- Weekend/on-call requirements: 1 weekend on call every 4th week (varies by location and profession).
- Dress code: Scrubs.
- Charting: Proficiency required in HomeCare HomeBase (HCHB) and OASIS (used within last 6 months or retraining required).
Required Qualifications
- Valid Registered Nurse (RN) license in Oregon.
- Minimum of 1 year of recent Home Health experience within the last year.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Current BLS certification issued by the American Heart Association or American Red Cross.
- Experience working with adult patients in a Home Health setting.
- Experience with OASIS documentation and processes.
Additional Requirements
- Must be willing to travel within the assigned territory (Salem to northeast including Canby, Molalla, Woodburn, Colton).
- Prior travel experience is not explicitly required.

- Various facility-specific compliance documents and training must be reviewed and signed prior to start (e.g., HIPAA, Code of Conduct, Privacy Policy, Home Health Scenarios, Compliance Training).
- Personal or auto insurance required for home health placements; policy must be current and under candidate’s name or list candidate as a driver.
