Job Opportunity: Registered Nurse - Home Health

Location: Salem, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
  • Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
  • Conduct home visits with periodic office meetings.
  • Average patient load: 5-8 patients per day.
  • Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
  • Must be willing to travel up to 75 miles from home clinic; average weekly travel approximately 250 miles.
  • Schedule: Monday to Friday, 8 AM to 5 PM.
  • Weekend/on-call requirements: 1 weekend on call every 4th week (varies by location and profession).
  • Dress code: Scrubs.
  • Charting: Proficiency required in HomeCare HomeBase (HCHB) and OASIS (used within last 6 months or retraining required).

Required Qualifications

  • Valid Registered Nurse (RN) license in Oregon.
  • Minimum of 1 year of recent Home Health experience within the last year.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
  • Current BLS certification issued by the American Heart Association or American Red Cross.
  • Experience working with adult patients in a Home Health setting.
  • Experience with OASIS documentation and processes.

Additional Requirements

  • Must be willing to travel within the assigned territory (Salem to northeast including Canby, Molalla, Woodburn, Colton).
  • Prior travel experience is not explicitly required.

  • Personal or auto insurance required for home health placements; policy must be current and under candidate’s name or list candidate as a driver.
