Registered Nurse (RN) - Long Term Care (LTC)
Location: Kankakee, Illinois
Contract Length: 13 weeks
Job Summary
Seeking an RN for a Long Term Care (LTC) setting in a skilled nursing facility (SNF) with approximately 80-95 residents. The role involves providing nursing care including IV therapy, assessments, medication administration, dressing changes, and wound vac management. The RN will handle admissions and discharges, participate in interdisciplinary team meetings, and may float between units.
Facility & Department Details
- Facility Type: Skilled Nursing Facility (SNF) / Long Term Care (LTC)
- Census: Typically 80-95 residents, with about 20 rooms not private
- Department: LTC / RN
- Nurse-to-Patient Ratios:
- RN LTC: 1:25
- RN SNF: 1:13
- LPN LTC: 1:25
- LPN SNF: 1:13
- CNA: 1:10-15 (aim for 1:10)
- EMR System: PCC
- Common Diagnoses: Feeding tubes, IV antibiotics, post-surgical care, rehabilitation, LTC conditions
- Respiratory Therapy: RT available during weekdays; nurses perform treatments otherwise
- Pharmacy: On-site 24/7 pharmacy with Omnicell and medication carts
Work Schedule & Requirements
- Shift Hours: 5 AM to 5:30 PM for RN/LPN; CNA shifts 8 or 12 hours
- Scheduling: Management-controlled; block scheduling possible
- Weekend Requirement: Every other weekend
- Holiday Requirement: Every other holiday
- Call/Standby: None
- Floating: May float between units but generally kept on the same unit during a stretch
- Orientation: One 12-hour shift on the floor; additional support if needed
- Scrub Color: Any professional scrub color allowed
- COVID Unit: Intermixed with other units
- Respiratory Protection: Use of PAPR or N95 masks
Patient Care & Duties
- IV Therapy: IV antibiotics and fluids; no titratable drips
- Assessments, medication administration, dressing changes, wound vacs
- Admission and discharge responsibilities
- Vital signs and ADLs performed by CNAs; no blood sugar monitoring by CNAs
- Participation encouraged in grand rounds and team meetings

- Physical Therapy (PT), Occupational Therapy (OT), Speech-Language Pathology (SLP)
- Respiratory Therapy (RT)
- Social Workers
- MDS Nurses
- Wound Care Nurse
- Dietician
Physician & Leadership Support
- Physician coverage after hours by hospitalist
- Nurse Practitioner available weekdays
- Some private MDs follow patients
- On-call provider available 24/7
- Leadership includes Assistant Directors of Nursing and Charge Nurses on all shifts
Required Qualifications
- Illinois RN license required and must be active at time of submission (temporary or permanent)
- BLS Certification required
- Driver’s license required for submission
- Minimum 1 year of RN experience, preferably with geriatrics
- First-time travelers are welcome
Additional Notes
- Local placements accepted if greater than 20 miles from the facility
- Radius rule applies for clinicians living within 50 miles (affects pay but not relevant here)
This position is ideal for RNs with LTC experience seeking a 13-week contract in a skilled nursing environment with a supportive interdisciplinary team and structured scheduling.
