Physical Therapist - Home Health
Location: Battle Creek, Michigan
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Summary
Seeking a Physical Therapist to provide home health care services. This is a full-time position with an 8:00 am - 5:00 pm shift, 40 hours per week. Weekend rotation is required every 4-6 weeks. No call or float requirements.
The therapist will evaluate, plan, and administer medically prescribed physical therapy treatments to patients with injuries or musculoskeletal conditions to improve function, strength, and independence. Direct patient care includes individual and group therapy activities, patient education, and interdisciplinary team collaboration.
Key Responsibilities
Clinical Duties
- Interpret and implement referrals; assess patient functional abilities (range of motion, strength, coordination) using standardized tests.
- Establish functional baselines and treatment goals.
- Participate in interdisciplinary rounds and team discussions.
- Provide patient, family, and community education.
- Conduct individual and/or group physical therapy sessions according to treatment plans.
- May design and fabricate splints and assistive devices.
- Participate in program development and departmental process improvement.
- Serve as a resource and provide in-services in areas of expertise.
- May cross-train and provide coverage in other rehabilitation departments as needed.
Administrative Duties
- Document evaluations, treatment plans, treatments performed, and patient progress in the electronic health record within compliance timelines.
- Communicate any limitations affecting timely documentation to leadership.
- Maintain accurate, legible, and complete patient records.
Required Qualifications
- Valid Physical Therapist license in Michigan
- Minimum of 1 year of Physical Therapist experience, including experience within the last year
- BLS certification issued by the American Heart Association
- Experience in a Home Health setting
Experience
- Specialty: Therapy/Rehabilitation (Physical Therapy)
- Required years of experience: Minimum 1 year, including recent experience in the last year
Additional Requirements
- Background check including criminal history, SSN trace, national sex offender, education verification, motor vehicle record, and others as required.
- MI LARA fingerprinting within 45 days of start.
- Drug testing: 10-panel including marijuana/THC within 30 days of start.
- Immunizations and health documents:
- Physical exam within 1 year of start.
- Negative TB test or chest x-ray as applicable.
- Proof of Tdap vaccine.
- Measles, Mumps, Rubella, Varicella immunizations or positive titers.
- Hepatitis B titer, series, or declination.
- Seasonal flu vaccination or approved exemption (for candidates starting after November 1).
- Completion of required compliance and facility-specific documents including code of conduct, appearance standards, and safety acknowledgments.
- Completion of competency exams and medication/pharmacology competency within 1 year prior to assignment start.
Work Environment
- Home health setting covering multiple counties.
- No float or call requirements.
- Weekend rotation every 4-6 weeks.
