Job Description

• Provide patient care for individuals requiring rehabilitation in an outpatient orthopedic setting.
• Focus on optimizing patients' physical, functional, psychological, social, and emotional well-being.
• Work schedule:

  • Monday, Wednesday: 9:30 AM – 6:00 PM
  • Tuesday, Thursday, Friday: 8:30 AM – 5:00 PM

Required Qualifications

• Massachusetts Physical Therapist License (MA PT License)
• Basic Life Support (BLS) certification
• Outpatient orthopedic experience is required

Onboarding & Compliance Requirements

  • Exclusion checks against federal and state healthcare exclusion lists
• Submission of resume covering past 7 years of work experience
• Two professional references, including at least one supervisor-level reference within the last 4 years
• Completion of skills checklist within 1 year of submission
• Confirmation of assignment form upon acceptance and for any extensions

Additional Preferred/Optional Health Requirements

• Hepatitis B vaccination or immunity proof (strongly preferred)
• Tdap vaccination within the last 10 years (preferred)
• If positive TB test, documentation of a negative chest x-ray

Notes

• MA Conditional Licenses are not accepted.
• Background checks and health documentation must be updated if there is a break in service longer than 6 months.
• All documentation must comply with Massachusetts Department of Public Health regulations.
