Job Opportunity: Physical Therapist - Home Health

Location: Glendale, California
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

• Work schedule options: Tuesday-Saturday or Wednesday-Sunday
• Local candidates are welcome to apply
• Home Health experience preferred; if no Home Health experience, outpatient experience is required
• OASIS experience required; candidates without OASIS experience may be considered if they have strong Home Health experience
• Hours are based on completed visits (25 visits = 40 hours); hours guaranteed if 25 or more visits are completed
• COVID exemptions permitted
• Mileage reimbursement follows IRS standard
• Candidates with a pending California license will be considered if the application has been submitted

Required Qualifications

• Valid Physical Therapist license in California
• Experience in an Outpatient setting
• OASIS experience

Additional Requirements

• Government-issued photo ID (e.g., driver’s license or passport)
• I9 attestation and background checks including 7-year work/school history and sex offender registry
• Professional references (2 required)
• Employment application and annual skills checklist
• Photo for badge (passport style)
• Personal or auto insurance documentation
• Auto registration documentation
• Completion and acknowledgment of training and hand hygiene policies

• Prior travel experience is not explicitly required for this position
• Candidates must be prepared to complete all required documentation and certifications prior to start
• This is a travel contract position with a 13-week duration
