Job Opportunity: Physical Therapist - Home Health

Location: South Portland, Maine
Employment Type: Travel/Contract
Contract Length: 13 weeks (approximately 3 months)

Job Description

• Provide physical therapy services in a home health setting.
• Conduct home visits and periodic office meetings.
• Average patient load: 5-6 patients per day.
• Types of cases: Start of Care and Follow Ups.
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM.
• Weekend rotation: None.
• On-call: None.
• Must be willing to travel up to 75 miles from the home clinic, averaging about 150 miles per week.
• Electronic Medical Record (EMR) system used: HCHB.
• Local candidates accepted.

Required Qualifications

• Valid Physical Therapist license in Maine.
• Minimum 1 year of recent experience as a Physical Therapist.
• Minimum 1 year of experience working with adult patients in a home health setting.
• Current BLS certification.
• Reliable transportation and a current driver’s license.
• Agency-required liability insurance.
• Must be willing to cover a large territory including multiple nearby towns.
• Must meet all background checks, drug screening, and health document requirements as per agency standards.

Additional Requirements & Compliance

• Must complete standard background checks including but not limited to:

  • 7-year work and residence history verification.
  • Sex offender registry checks.
  • State-specific criminal background checks.
  • Drug screening (10-panel).
• Health documents required:

  • Physical exam within 12 months.
  • Negative TB test or chest X-ray within 12 months.
  • Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
  • Flu vaccination or documented declination.
  • COVID-19 vaccination or documented declination.
• Must review and sign various compliance, confidentiality, and policy documents prior to start.
• Mileage reimbursement provided at IRS standard rate for travel between patient homes (commute to/from first and last patient not reimbursed).

Notes

• Weekend and on-call requirements may vary by location; discuss during interview.
• Schedule may vary in some territories (e.g., 4x10 shifts Friday-Monday), but most are standard 5x8.
• This position is created due to a permanent staff member on maternity leave.
• Candidates must be prepared to meet all state-specific licensing and background requirements.

This opportunity is ideal for experienced physical therapists with recent home health experience who are comfortable traveling within a large service area and working independently in patients' homes.
