Job Opportunity: Speech-Language Pathologist (SLP) - Home Health
Location: Alameda, California
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Summary
Provide speech therapy services in a home health setting, focusing on treatment plans for speech and swallowing disorders. Responsibilities include patient evaluations, developing and updating plans of care, treatment progression, documentation, and coordination with care teams. May also involve clinical supervision and instruction of assistants and students.
Key Responsibilities
- Develop and implement treatment plans for speech and swallowing disorders (e.g., stuttering, delayed language).
- Conduct patient evaluations and testing per Speech Pathology Standards of Practice.
- Update and manage patient Plans of Care (POC).
- Provide and progress treatment according to established standards.
- Complete all required documentation accurately and timely.
- Coordinate patient care with other providers and support team members.
- Work flexible shifts including varied days and hours.
- Assist with clinical supervision of assistants and aides.
- Serve as a clinical instructor for assigned students.
- Maintain a clean, safe working environment and follow equipment use protocols.
- Participate in quality improvement and interdisciplinary projects.
- Ensure compliance with patient privacy (PHI) and regulatory requirements.
- Maintain certifications and complete continuing education as required.
Required Qualifications
- Valid Speech-Language Pathologist license in California
- Current State License to practice as a Speech-Language Pathologist in California
- Certification: SLP Clinical Fellowship (SLP-CF)
- Basic Life Support (BLS) certification
Background and Compliance Requirements
- 7-year background check including SSN trace, county searches, sex offender search, education verification, and motor vehicle report (if applicable).
- FACIS Level 3 screening required within 2 weeks of start and monthly thereafter.
- Annual updates for background checks and certain screenings.
- Education verification and transcript/diploma review as needed.
- Drug screening: Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids per California law).
- Physical exam within 1 year of start.
- Fit mask test within 1 year, renewed annually (with accommodations considered for religious exemptions or facial hair).
- Immunizations and titers required: TDAP, Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella.
- Negative TB test or chest x-ray within 90 days of start, renewed annually.
- Seasonal flu vaccination or approved declination during flu season.
- Respiratory safety clearance prior to start, renewed annually.
- References: 2 professional references within 3 years (Aya requirement).
- Full 7 years of work history required on application with verification of employment.
Additional Notes
- Flexible shift assignments may be required.
- May perform some clerical duties related to patient records and appointments.
- Must maintain strict confidentiality of patient information.
- Candidate must review and sign facility-specific policies and conduct standards prior to start.
- Religious exemptions for certain requirements are considered on a case-by-case basis with appropriate documentation.
