Registered Nurse (RN) – Long Term Care (LTC)

Location: Kankakee, Illinois
Contract Length: 13 weeks

Facility & Department Details

• Facility Type: Skilled Nursing Facility (SNF) and Long Term Care (LTC)
• Census: Typically 80-95 residents
• Room Setup: Approximately 20 rooms are not private
• Department: LTC / RN
• Electronic Medical Record (EMR) System: PCC
• Nurse-to-Patient Ratios:

  • RN LTC: 1:25
  • RN SNF: 1:13
  • LPN LTC: 1:25
  • LPN SNF: 1:13
  • CNA: 1:10-15 (aim to keep around 10)

Job Responsibilities

• Administer IV antibiotics and fluids (no titratable drips)
• Perform patient assessments, medication administration, dressing changes, and wound vac care
• Complete admissions and discharges
• Participate in grand rounds and team meetings (encouraged)
• Float between units possible but preference to stay on the same unit during a stretch
• Collaborate with interdisciplinary team including PT/OT/SLP, respiratory therapists, social workers, MDS nurses, wound care nurse, and dieticians
• Use of PAPR and N95 masks as needed
• Orientation includes one 12-hour shift on the floor to assess comfort and confidence

Patient Population & Common Diagnoses

• Geriatric patients
• Common conditions: feeding tubes, IV antibiotics, post-surgical care, rehabilitation, long-term care needs
• Respiratory support includes AVAP/CPAP/BIPAP, mostly at night

Staffing & Support

• Respiratory therapist available during daytime Monday-Friday; nurses provide treatments otherwise
• Physician coverage: hospitalist after hours, nurse practitioner during weekdays, some private MDs, and 24/7 on-call provider
• Pharmacy: Dedicated on-site pharmacy available 24/7 with Omnicell and medication carts

Scheduling & Requirements

• Weekend and holiday work: every other weekend/holiday required
• No call or standby requirements
• Scrub color: Any professional color accepted
• Drivers license required for submission
• Local placements accepted if greater than 20 miles from facility

Required Qualifications

• Illinois RN license required and must be in hand to submit (temporary or permanent)
• BLS certification required
• Minimum 1 year of RN experience, preferably with geriatrics
• First-time travelers are welcome

Additional Notes

• COVID patients are intermixed with other residents
• Radius rule: Clinicians living within 50 miles of the facility are offered a reduced rate (not included here)
