RN Case Manager Position - Indiana, Indianapolis

Job Summary

  • Coordinate the overall interdisciplinary plan of care for patients from admission to discharge.
  • Develop care plans across the continuum, integrating patient and family preferences and values.
  • Monitor patient medical necessity and level of care through assessments, ongoing evaluations, and patient records.
  • Advocate for resources and removal of barriers to care.
  • Maintain ongoing communication with supervisors and care transition team members to ensure effective implementation and reevaluation of health plans.
  • Serve as a resource for medical record documentation, level of care recommendations, and post-discharge care services related to diagnoses and treatment options.

Contract Length

  • 13 weeks

Shift

  • 8:00 AM to 4:30 PM

Required Qualifications

  • Valid RN license in Indiana
  • Minimum of 2 years of Case Management experience, including experience within the last year
  • Basic Life Support (BLS) certification issued by the American Heart Association
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Onboarding and Compliance Requirements

  • Signed assignment agreement and confirmation documents.
  • Completion of core competencies including HIPAA, Joint Commission, NPSG, and OSHA topics with a minimum passing score.
  • Completion of unit-specific checklist (optional).
  • Completion of onboarding education attestation and nursing contract worker onboarding.
  • Forward-facing badge photo with white or blue background (no alterations).
  • Health and immunization requirements including:
    • MMR: documented proof of two vaccines or positive IGG titer; no declinations accepted.
    • Varicella: documented proof of two vaccines, serological immunity, or verified history; no declinations accepted.
    • Tdap: valid within 10 years; no declinations accepted.
    • Hepatitis B: vaccination series completion or signed declination.
    • Influenza vaccine (optional; mask required if declined during flu season).
    • Fit test for respiratory protection, preferably completed prior to start.
    • Tuberculosis screening: 2-step PPD or IGRA testing, chest X-ray if positive, and annual TB questionnaire.
    • Statement of good health or physical exam prior to first assignment, updated annually.
  • Background and screening:
    • Criminal background check including Social Security Trace, OIG Search, National Sex Offender Registry, and other required checks within 30 days of assignment start.
    • Employment verification for the past seven years.
    • Drug screening: 12-panel drug test within 30 days prior to start, updated annually.
    • Education verification relevant to clinical work within 90 days of start.
    • EPLS (Excluded Parties List System) check within 90 days of start.
  • Skills checklist for clinical specialty completed within one year of start and updated annually.
  • Confirmation of assignment documentation upon initial assignment and each extension.
