Physical Therapist Position - Oregon

Location

• City: Baker City, Oregon

Employment Type

• Long-Term Assignment (LTA)
• Shift: 7:30 AM – 5:00 PM
• Days with occasional Saturday coverage (typically 2 Saturdays per 13-week contract)
• Primarily outpatient care

Contract Length

• 13 weeks

Required Qualifications

• Valid Physical Therapist license in the state of Oregon
• Basic Life Support (BLS) certification from the American Heart Association (front and back of card required)
• Specialty-specific competency testing completed within the past year
• CORE exam within the past year
• Physical/Health statement confirming fitness to work (exam within the last year)
• Drug screening: 10-panel drug screen completed within 30 days of start (no THC)
• Fit test required
• Immunizations and health requirements:

  • COVID-19 vaccine booster or approved declination/exemption (valid through October 2026)
  • MMR vaccination or immunity proof
  • Hepatitis B vaccination series (titers not accepted)
  • Influenza vaccine or declination for current and following flu seasons
  • Varicella vaccination or immunity proof
  • 2 Step PPD or IGRA (Quantiferon/Tspot) test for TB screening
• Motor Vehicle Report (MVR) dated within 30 days of start (if applicable)
• Criminal background check (federal, nationwide, county) within last 7 years, including SSN verification and alias checks, dated within 30 days of start
• Education verification (highest level of academic education)
• Various professional compliance forms and agreements (code of conduct, confidentiality, integrity and audit services, conflict of interest disclosure, system access form)
• Exclusion screenings (EPLS, OIG/GSA, NSO) dated within 30 days of start, including alias checks
• Skills checklist and profile including resume, skills checklist within a year, and 2 professional references
