Physical Therapist - Home Health

Location: Battle Creek, Michigan
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Summary

Provide medically prescribed physical therapy treatment for patients with injuries or muscle, nerve, joint, and bone diseases to improve function, increase strength, relieve pain, and enhance independence. Must have home health experience.


Schedule

  • Shift: 8:00 AM - 5:00 PM
  • Hours: 40 hours/week (Full Time)
  • Weekend Requirement: Every 4th weekend
  • Call Requirement: No
  • Float Requirement: No

Clinical Duties

  • Interpret and implement referrals
  • Evaluate patient functional abilities/disabilities (range of motion, strength, coordination) using standardized assessments
  • Establish functional baselines and treatment goals
  • Participate in interdisciplinary team discussions and rounds
  • Provide patient, family, and community education
  • Conduct individual and/or group physical therapy activities per treatment plan
  • May design and fabricate splints and assistive devices
  • Participate in program development and department process improvement
  • Serve as a resource and provide in-services in areas of expertise
  • May cross-train and provide coverage in other rehabilitation departments as needed

Administrative Duties

  • Document evaluations, treatment plans, treatments performed, and patient progress in the electronic health record timely and accurately
  • Communicate any limitations preventing timely documentation to leadership

Required Qualifications

  • Valid Physical Therapist license in Michigan
  • BLS issued by American Heart Association
  • Experience in a Home Health setting

Required Documentation & Compliance

  • Criminal background check (lifetime search, including SSN trace, national/statewide searches, education verification, MVR for home health) within 45 days of start
  • MI LARA fingerprinting within 45 days of start (required for home health positions)
  • Physical exam within 1 year of start
  • Drug testing: 10-panel including Marijuana/THC within 30 days of start
  • Immunizations and health requirements:
    • Tetanus/Diphtheria/Tdap (proof of at least one prior Tdap vaccine)
    • Negative TB test or chest x-ray as applicable
    • Measles, Mumps, Rubella (2 vaccines or positive titer)
    • Hepatitis B (titer, series, or declination)
    • Varicella (2 vaccines or positive titer)
    • Seasonal flu vaccination or approved exemption (for candidates starting after November 1)
  • Proof of personal or auto insurance (updated every 6 months)
  • Completion of required forms such as I-9, E-Verify, and code of conduct acknowledgments
  • References: One professional reference from a supervisor or manager required
  • Employment application/resume with full 7 years work history, no gaps

Additional Notes

  • No call or float requirements
  • Coverage area includes multiple counties in Michigan
  • Candidates must complete fingerprint consent and disclosure forms promptly to schedule fingerprinting appointments
  • Fit testing may be required depending on assignment location and role
