Position Overview

Job Responsibilities & Requirements

• Valid Physical Therapist license in Indiana
• Minimum 1 year of Physical Therapist experience within the last year
• Experience in an Outpatient setting

Dress Code

• Scrubs color: Charcoal gray
• No denim allowed
• No extreme hairstyles or unsafe facial jewelry
• Visible tattoos must be respectful or covered
• Hats/head coverings only for religious or medical accommodations
• Socks/stockings/footies/nylons required
• Shoes must be clean, closed-toe, solid surface without holes, and easy to clean

Additional Information

• CBTs and EMR training provided upon arrival
• Reporting location is the outpatient therapy building front entrance
• Parking available behind the building and between the building and hospital
