• Day shift, 9:00 AM to 5:00 PM
• 40 hours per week (5 days x 8 hours)

Required Qualifications

• Valid Physical Therapist license in California
• One completed travel assignment
• Basic Life Support (BLS) certification
• Experience in a Home Health setting

Background and Compliance Requirements

• 7-year background check covering all aliases, county searches, nationwide sex offender search, education verification, and motor vehicle report (required for driving positions)
• FACIS Level 3 screening within 2 weeks of start, with monthly updates thereafter
• Annual updates for EPLS/GSA/SAM, HHS/OIG, and other regulatory checks
• Education verification of highest level achieved (not required if highest education is high school)
• Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids due to California law)
• TB screening questionnaire within 90 days of start, renewed annually
• Negative TB test (QGold, TSPOT, or PPD) within 90 days of start, renewed annually
• Physical exam within 1 year of start, including clearance for work
• Fit mask test within 1 year of start, renewed annually (with accommodations for religious exemptions or failed tests)
• Immunizations and/or titers required:

  • Tetanus/Diphtheria/Tdap (within 10 years)
  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B (series or declination)
  • Varicella
  • Seasonal flu vaccination during flu season (Nov 1 - Apr 30), with mask requirement if declined
• Respiratory safety clearance prior to start, renewed annually

Documentation and Verification

• Full 7 years of work history with no gaps, including city and state for all locations worked
• Employment verification for up to 7 years, with explanations for gaps over 30 days
• Two professional references within 3 years (clinical/allied/per diem acute care)
• Unit-specific skills checklist, renewed annually
• Primary source license verification within 30 days of start
• Copy of professional license if verification does not include license number

Facility-Specific Requirements

• Personal or auto insurance effective for the duration of the assignment (required for home health and driving positions)
• Completion and signing of facility-specific documents prior to start, including:

  • Job description acknowledgment
  • Standards of business conduct
  • Code of conduct
  • Overpayment refund policy
  • Vendor training (2024 and 2025 versions depending on contract date)
  • Conditional offer of employment attestation
  • Religious accommodation forms if applicable
  • TB questionnaire signed by candidate and healthcare provider (MD, PA, NP/APN, DO, or RN)
• Photo for badge (per diem only)

Certifications (as applicable)

• ACLS, PALS, NRP, CCRN, TNCC, EFM, CPI, NIH Stroke Certification, and others may be required depending on position specifics (verify with manager)
