Physical Therapist - Home Health

Location: Ventura, California
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

• Field position providing home health physical therapy services.
• Schedule options include:

  • Monday to Friday with one weekend day on call per month
  • Tuesday to Saturday
  • Wednesday to Sunday
• Local candidates are welcome.
• Hours are based on completed visits: 25 visits equal 40 hours per week. Hours are guaranteed if 25 or more visits are completed.
• Mileage reimbursement follows IRS standard rates.
• COVID vaccination exemptions permitted.

Required Qualifications

• Valid Physical Therapist license in California

Certifications & Compliance

• Basic Life Support (BLS) - AHA Instructor or Provider acceptable, required for all.
• Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Neonatal Resuscitation Procedure (NRP), and Electronic Fetal Monitoring (EFM) as applicable.
• Occupational Therapists are not required to have NBCOT certification.
• Primary source online verification of state license required.
• Medication/Pharmacology competency exams required as applicable, with minimum passing scores.

Documentation & Screening Requirements

• Government-issued photo ID (Driver’s License or State ID).
• Valid passport copy before start.
• I9 attestation and employment application required.
• Standard background check including 7-year residential/work/school history, sex offender registry, and annual EPLS/GSA/SAM and HHS/OIG checks.
• Physical exam within 12 months of start date.
• Immunizations and health screenings:

  • TDAP vaccine within past 10 years (no declinations accepted).
  • Negative TB test or chest X-ray within 1 year; if past positive TB, annual TBQ and chest X-ray within 5 years required.
  • Proof of immunity or vaccination for Measles, Mumps, Rubella (MMR) for candidates born in 1957 or later (no declinations accepted).
  • Hepatitis B titer or vaccine series; declination accepted if within 1 year of start.
  • Varicella immunity or vaccination (no declinations accepted).
  • Negative 9-panel drug screen within 1 year of start.
  • Seasonal flu vaccination or declination accepted.
  • COVID vaccination proof or medical/religious exemption required prior to start.

Additional Requirements

• Passport-style photo for badge (shoulders up, front facing, plain background).
• Personal or auto insurance with current expiration dates.
• Auto registration documentation required.
• Completion and acknowledgment of training content and hand hygiene policy prior to start.
• Two professional references required.
• Annual skills checklist required.

Shift & Hours

• Day shift, typically 8:00 AM to 5:00 PM (5 days per week, 8 hours per day).
• 40 hours per week based on visit completion.

Notes

• Candidates without a current California license may be accepted if the license application is in process.
• COVID exemptions are allowed.
• Mileage reimbursement is provided for travel between patients.
