Description
Physical Therapist - Home Health (Travel/Contract)
Location: Victorville, California
Contract Length: 13 weeks
Job Description
- Field position covering Hesperia, Victorville, and High Desert areas.
- Home Health setting with schedule options:
- Monday to Friday with one weekend day on call per month
- Tuesday to Saturday
- Wednesday to Sunday
- Mileage reimbursement follows IRS standard rates.
- Guaranteed hours based on completed visits: 25 visits = 40 hours/week.
- COVID vaccination proof or medical/religious exemption required.
- Candidates with pending California license will be considered if application is submitted upon submission.
Required Qualifications
- Valid Physical Therapist license in California
- Experience in an Outpatient setting
- Experience with OASIS
Required Documentation
- Government-issued photo ID (Driver’s License or State ID).
- Valid passport copy before start.
- I9 attestation for clearance.
- Background checks including 7-year location history, EPLS/GSA/SAM, and HHS/OIG (updated annually).
- Photo for badge (passport style, shoulders up, front facing, plain background).
- Personal or auto insurance with current expiration dates.
- Auto registration with current expiration dates.
- Signed acknowledgments for training content and hand hygiene policy.
- Employment application and annual skills checklist.
- Medication/pharmacology competency exam as applicable (minimum 80% or meet Angoff score).
- State license primary source online verification.
Shift Details
- Day shift, 5 days per week, 8 hours per day (8:00 AM to 5:00 PM).
- Total 40 hours per week.
Additional Notes
- Locals are welcome to apply.
- COVID exemptions permitted.
- Hours guaranteed if 25 or more visits are completed weekly.
