Physical Therapist - Home Health (Travel/Contract)

Location: Baldwin Park, California
Contract Length: 13 weeks

Job Description

• Field position providing home health physical therapy services.
• Schedule options include:

  • Monday to Friday with one weekend day on call per month
  • Tuesday to Saturday
  • Wednesday to Sunday
• Locals are eligible to apply.
• Hours are based on completed visits: 25 visits equal 40 hours per week. Hours are guaranteed if 25 or more visits are completed.
• Mileage reimbursement follows IRS standard rates.
• COVID vaccination exemptions are permitted.
• Candidates with a pending California license will be considered if the application has been submitted.

Required Qualifications

• Valid Physical Therapist license in California.
• Experience in an outpatient setting.
• Experience with OASIS.

Shift & Hours

• Day shift, typically 8:00 AM to 5:00 PM (5 days a week, 8 hours per day).

Required Documentation & Compliance

• Government-issued photo ID (Driver’s License or State ID).
• Valid passport copy before start.
• I9 attestation for employment eligibility.
• Standard background check including 7-year search and nationwide sex offender check.
• Annual EPLS/GSA/SAM and HHS/OIG checks.
• Certifications as applicable:

  • BLS (Basic Life Support) - AHA Instructor or Provider accepted, required for all.
  • ACLS, PALS, NRP, EFM as applicable (verify based on clinical level).
  • Occupational Therapists are not required to have NBCOT certification.
• Primary source online verification of state license.
• Medication/Pharmacology competency exam with minimum passing score of 80% or Angoff score.
• Competency exam as applicable with minimum passing score of 80% or Angoff score.
• Passport-style photo for badge (shoulders up, front facing, plain background).
• Personal or auto insurance with current expiration dates.
• Auto registration documentation with current expiration dates.
• Review and sign training content and hand hygiene policy acknowledgments prior to start.

Health & Immunization Requirements

• Physical exam within 12 months of start date.
• TDAP vaccine within the past 10 years (declinations not accepted).
• Negative TB test (PPD, QGold, or TSpot) within 1 year; if past positive TB, annual TBQ and chest x-ray within 5 years required.
• Proof of immunity or vaccination for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Varicella (Chicken Pox)
• Hepatitis B titer or vaccine series accepted; declination accepted if within 1 year of start.
• Negative 9-panel drug screen within 1 year of start (instant drug screen acceptable).
• Seasonal flu vaccination/documentation required year-round (declinations accepted).
• COVID vaccination proof or medical/religious exemption required prior to start.

Additional Requirements

• Two professional references required.
• Employment application must be completed and submitted for clearance.
• Annual skills checklist required.
