Description
Radiation Therapist Position - Idaho, Nampa
Job Details
- Employment Type: Long-Term Assignment (LTA)
- Contract Length: 13 weeks
- Schedule: Monday to Friday, 8:00 AM to 5:00 PM
- Hours: 36-40 hours per week
- No call or holiday shifts required
- High possibility of contract extension after initial assignment
Required Qualifications
- Radiation Therapist with at least 1 year of experience, including experience within the last year
- Valid Radiation Therapist license for Idaho, verified within 30 days of start
- BLS certification issued by the American Heart Association (must provide front and back card images)
- Fit test for N95 masks (3M brand models specified) required; onsite fit testing may be approved if necessary
- Completion of a skills checklist provided by the agency prior to start
- Must pass a 10-panel drug screen within 30 days of start (including methadone, phencyclidine, benzodiazepines, cocaine, propoxyphene, amphetamines, opiates, 6-acetyl morphine, methaqualone, barbiturates)
- Current physical/health statement confirming fitness to work (exam within last year)
- Up-to-date immunizations and health screenings:
- MMR: Proof of 2 vaccinations or IgG titer lab report
- Hepatitis B: At least first vaccination prior to start; full series required
- 2-Step PPD or IGRA TB test within 30 days of start; if positive, chest x-ray and medical evaluation required
- Varicella: Proof of 1 vaccination or IgG titer lab report
- Tdap: Current vaccine or accepted declination if no pediatric interaction
- COVID-19 vaccine with booster proof or approved declination/exemption (declination not accepted in CA or IL)
- Criminal background check (federal, nationwide, county for last 7 years) and SSN verification within 30 days of start
- Motor Vehicle Report (MVR) dated within 30 days of start if applicable
- Education verification of highest academic degree or diploma
- Completion of required professional forms including:
- Code of Conduct
- Confidentiality and Information Security Agreement
- Integrity and Audit Services Form
- Conflict of Interest Disclosure
- Criminal Background Check
- EPLS and GSA/OIG Exclusion Reports
- Joint Commission CORE Exam within the last year
- National Sex Offender Registry (NSO) check within 30 days
- System Access Form completed one week before start
- Standard I-9 form completed by agency
