Physical Therapist Home Health - Travel/Contract

Location: Dublin, Ohio

Job Details

• Shift: Day shift, Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours)
• Hours per Week: 40 hours
• Average Patients per Day: 5-6
• Patient Visits: Start of Care and Follow Ups
• Travel Requirements: Must be willing to travel up to 75 miles from home clinic; approximately 200 miles traveled per week.
• Work Setting: Home Health (home visits with periodic office meetings)
• EMR System: HCHB

Required Qualifications

• Valid Physical Therapist license in Ohio
• Minimum 1 year of recent Physical Therapist experience within the last year
• Experience working with adult patients in a Home Health setting
• BLS certification

Schedule and Work Conditions

• Typical schedule is Monday through Friday, 8 AM to 5 PM (standard 5x8s)
• Some positions may have alternate schedules such as 4x10s (Friday-Monday) depending on territory
• Weekend and on-call requirements vary by location and should be discussed during the interview
• No guaranteed hours; if patient census is low, floating to a nearby clinic may be arranged

Compliance and Documentation Requirements

• Background checks covering the last 7 years, including SS trace, county, nationwide, sex offender, and state-specific searches
• Fingerprinting required within 12 months prior to start and updated annually (Ohio BCI/FBI fingerprinting included)
• Education verification for degree applicable to Physical Therapy
• Drug screening: 10-panel drug test required
• Health documents required prior to start and updated annually:

  • Physical exam within 12 months confirming good health and no communicable disease
  • Negative TB test or chest X-ray within 12 months
  • Immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella
  • Flu vaccination or documented declination for current season
  • COVID vaccination series or documented declination
• Two professional references required
• Work history verification for at least the last 7 years or two most recent employers

Additional Requirements

• Must review and sign various compliance, confidentiality, privacy, and policy documents prior to start and annually if working more than one year consecutively
• Personal or auto insurance required for home health placements; policy must be under candidate’s name or list candidate as a driver
• Must complete agency-required compliance training and quizzes prior to start

Notes

• Mileage is reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient is not billable)
• Local candidates accepted at the same rate
• On-call and weekend requirements vary and should be clarified during interview

This position is ideal for experienced Physical Therapists with recent adult home health experience who are comfortable with travel and home visits in the Ohio area.
