Physical Therapist - Outpatient (Travel/Contract)
Location: San Francisco, California
Contract Length: 13 weeks
Job Description
- Neurological/orthopedic physical therapist role in an outpatient setting
- Work schedule: Monday to Friday, 9:00 AM to 5:30 PM (5 days x 8 hours)
- No on-call, weekend, or floating duties
Required Qualifications
- Valid Physical Therapist license in California
- Minimum 2 years of Physical Therapist experience, including experience within the last year
- Experience in an outpatient setting
- Basic Life Support (BLS) certification (AHA or ARC) required
Background and Compliance Requirements
- Comprehensive background checks including:
- Social Security Trace with all aliases
- County searches for last 7 years
- Nationwide Sex Offender Search within 90 days of start
- FACIS Level 3 screening within 2 weeks of start and monthly thereafter
- EPLS/GSA/SAM and HHS/OIG checks updated annually
- Education verification of highest level achieved (not required if highest education is high school)
- Employment verification for up to 7 years, with explanations for gaps over 30 days
- References: 2 professional references within last 3 years required
Health and Safety Requirements
- Respiratory safety clearance completed prior to start and renewed annually (not required for some non-clinical roles)
- Color vision exam by licensed healthcare provider if applicable
- Drug testing excludes marijuana/cannabinoids due to California law effective 1/1/24
Additional Notes
- Candidates with religious exemptions for vaccines or facial hair must complete specific exemption forms and receive approval
- Candidates must review and sign facility policies including standards of business conduct, code of conduct, vendor training, and regulatory orientation prior to start
