Occupational Therapist, Outpatient
Location: Northampton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift:
- Monday & Wednesday: 9:00 AM – 5:30 PM
- Tuesday & Friday: 8:00 AM – 4:30 PM
- Thursday: 8:30 AM – 5:00 PM
Job Description
Provide outpatient occupational therapy focused on hand and neurological rehabilitation. Deliver patient care tailored to optimize physical, functional, psychological, social, and emotional well-being. Work under clinical supervision within established policies and procedures.
Required Qualifications
- Valid Occupational Therapist license in Massachusetts
- BLS issued by American Heart Association or Red Cross
- A Bachelor of Science degree in Occupational Therapy is mandatory
Onboarding and Compliance Requirements
- Submit a recent (within 6 months) professional color photo (no selfies, plain background, no eyeglasses)
- Professional license verification with primary source verification (no conditional licenses accepted)
- Documentation of immunizations or immunity for MMR (measles, mumps, rubella)
- TB screening documentation within 3 months prior to start (skin test or blood test)
- Physician health statement within 1 year of start date
- Criminal background check within 45 days prior to start, including Massachusetts CORI and national sex offender registry checks
- Exclusion checks against OIG, GSA, MassHealth, and NH Medicaid lists within 30 days prior to start and annually
- Verification of highest level of education achieved
- Social Security Number trace for the past 7 years
- Submission of a detailed resume covering the past 7 years of work experience (including city and state)
- Two professional references within the last 4 years (minimum one supervisor-level)
- Completion of a skills checklist within 1 year of submission
- Confirmation of assignment form upon acceptance and for any extensions
- Candidate submission coversheet for initial assignment and new facility submissions
Additional Notes
- Positive TB test requires a negative chest x-ray dated after the positive test
- Name change documentation required if credentials do not match current legal name
- TB symptom survey required if positive PPD, updated annually
- Prior travel experience is not specified as required for this position
