Description
Physical Therapist - Home Health (Travel/Contract)
Location: Demopolis, Alabama
Contract Length: 13 weeks
Job Description
- Provide physical therapy services in a home health setting with home visits and periodic office meetings.
- Average patient load: 5-6 patients per day, with 5-10 patients assigned daily equating to roughly 6-8 points per day (30 points expected weekly).
- Types of cases: Start of Care and Follow Ups.
- Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hour shifts). Weekend and on-call requirements vary and should be discussed during the interview.
- Must be willing to travel up to 75 miles from the home clinic, with an estimated 500 miles traveled weekly. Mileage reimbursed at IRS standard rate (patient-to-patient travel compensated; commute to/from first and last patient is not billable).
- Local candidates accepted at the same rate.
Required Qualifications
- Valid Physical Therapist license in Alabama
- Minimum of 1 year of experience as a Physical Therapist within the last year
- Experience working with adult patients
- Experience in a Home Health setting
- Current BLS certification
Productivity & Systems
- Weekly productivity expectation: 30 points.
- Electronic Medical Record (EMR) system used: Home Care Home Base.
Compliance & Documentation Requirements
- Must complete and maintain all required background checks, including but not limited to:
- 7-year standard background check with SS trace, county, nationwide, sex offender, and state-specific searches.
- EPLS/GSA/SAM and HHS/OIG checks within 14 days of start and monthly thereafter.
- Fingerprinting as required by state regulations.
- Education verification required for degree applicable to physical therapy.
- Two professional references required.
- Work history verification for at least the last 7 years or two most recent employers.
- Physical exam within 12 months prior to start confirming good health and absence of communicable disease symptoms.
- Negative TB test or chest X-ray within 12 months, updated annually.
- Immunization or titer proof for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella per standard requirements.
- 10-panel drug screen required.
- Seasonal flu vaccination or documented declination required.
- COVID-19 complete vaccination series or declination required.

- COVID-19 Policy and Attestation
- TB Questionnaire
- Other state-specific and facility-required forms as applicable
