Physical Therapist - Home Health (Travel/Contract)

Location: Demopolis, Alabama
Contract Length: 13 weeks

Job Description

• Provide physical therapy services in a home health setting with home visits and periodic office meetings.
• Average patient load: 5-6 patients per day, with 5-10 patients assigned daily equating to roughly 6-8 points per day (30 points expected weekly).
• Types of cases: Start of Care and Follow Ups.
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hour shifts). Weekend and on-call requirements vary and should be discussed during the interview.
• Must be willing to travel up to 75 miles from the home clinic, with an estimated 500 miles traveled weekly. Mileage reimbursed at IRS standard rate (patient-to-patient travel compensated; commute to/from first and last patient is not billable).
• Local candidates accepted at the same rate.

Required Qualifications

• Valid Physical Therapist license in Alabama
• Minimum of 1 year of experience as a Physical Therapist within the last year
• Experience working with adult patients
• Experience in a Home Health setting
• Current BLS certification

Productivity & Systems

• Weekly productivity expectation: 30 points.
• Electronic Medical Record (EMR) system used: Home Care Home Base.

Compliance & Documentation Requirements

• Must complete and maintain all required background checks, including but not limited to:

  • 7-year standard background check with SS trace, county, nationwide, sex offender, and state-specific searches.
  • EPLS/GSA/SAM and HHS/OIG checks within 14 days of start and monthly thereafter.
  • Fingerprinting as required by state regulations.
• Education verification required for degree applicable to physical therapy.
• Two professional references required.
• Work history verification for at least the last 7 years or two most recent employers.
• Physical exam within 12 months prior to start confirming good health and absence of communicable disease symptoms.
• Negative TB test or chest X-ray within 12 months, updated annually.
• Immunization or titer proof for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella per standard requirements.
• 10-panel drug screen required.
• Seasonal flu vaccination or documented declination required.
• COVID-19 complete vaccination series or declination required.

• Guaranteed hours are not provided; if patient census is low, floating to a nearby clinic may be discussed.
