Physical Therapist - Outpatient Rehab (Travel/Contract)
Location: Crescent City, California
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Overview
Seeking a full-time Physical Therapist in a hospital setting with a primary focus on Outpatient Rehabilitation and some Inpatient coverage.
Job Duties
- Assess, plan, organize, and participate in rehabilitative programs to improve patient mobility, relieve pain, increase strength, and correct disabling conditions from disease or injury.
- Develop and update individualized physical treatment programs and patient Plans of Care (POC).
- Complete and document patient evaluations, tests, and all related therapy documentation per regulatory and departmental standards.
- Provide and progress treatment according to the Plan of Care.
- Coordinate patient care with other healthcare providers and support team members.
- Provide clinical supervision to assistants and aides; serve as clinical instructor for assigned students.
- Participate in quality improvement processes and interdisciplinary projects.
- Maintain a clean, safe working environment and ensure proper use and inspection of equipment.
- Adhere to all local, state, and federal regulations, including patient privacy and safety standards.
Schedule
- Day shift, 4 days per week, 10 hours per day (8:00 AM – 4:30 PM)
Required Qualifications
- Valid Physical Therapist license in California
- Basic Life Support (BLS) Certification
Background and Compliance Requirements
- 7-year background check required within 6 months of start, including SSN trace, county searches, sex offender search, education verification, and FACIS Level 3 screening.
- Annual updates required for EPLS/GSA/SAM, HHS/OIG, and FACIS screenings.
- Education verification required (not needed if highest education is high school).
- Must maintain strict confidentiality of all patient Protected Health Information (PHI).
Health and Immunization Requirements
- Physical exam within 1 year of start, including clearance for work and any necessary accommodations.
- Fit mask test within 1 year, renewed annually (exceptions apply).
- Adult Tetanus/Diphtheria/Tdap vaccination within 10 years or lifetime TDAP with booster within 10 years.
- Negative TB test or chest X-ray within 90 days of start, renewed annually.
- Proof of immunity or vaccination for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (series or signed declination)
- Varicella
- Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids per CA law).
- Seasonal flu vaccination or signed declination during flu season (Nov 1 – Apr 30).
Documentation and Verification
- Must provide full 7 years of work history with no gaps, including city and state of employment.
- Employment verification required for up to 7 years; gaps over 30 days must be explained.
- Two professional references required within 3 years for clinical/allied/per diem acute care candidates.
Additional Notes
- Candidate must review and sign facility-specific documents including standards of business conduct, code of conduct, vendor training, and regulatory orientation prior to start.
- Religious exemptions for certain immunizations and fit mask requirements may be considered on a case-by-case basis with appropriate documentation.
- Clinical supervision and instructor responsibilities included in role.
