Description

Registered Nurse - Home Care

Job Description

Provide case management for home health patients.
Manage a high volume caseload, seeing 5-6 patients per day.
Extensive documentation required.
Work with the South team covering Plainsboro and West Windsor areas.

Contract Length

13 weeks

Schedule

Shift: 8:00 AM – 4:30 PM
Hours per week: 40

Required Qualifications

Valid Registered Nurse (RN) license in New Jersey.
Associate of Science in Nursing or Associate Degree in Nursing.
Minimum 2 years of home health experience, including experience within the last year.
Experience in a home health setting.
Case management experience.
Experience with OASIS (Outcome and Assessment Information Set).
BLS certification.
