Physical Therapist - Inpatient Rehab
Location: Crescent City, California
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Overview
Seeking a full-time Physical Therapist in a hospital setting with a focus on acute care, inpatient rehab, and outpatient rehab. The schedule is Friday through Tuesday, with the ability to float between inpatient, acute rehab unit (ARU), and outpatient as needed.
Key Responsibilities
- Assess, plan, organize, and participate in rehabilitative programs to improve patient mobility, relieve pain, increase strength, and correct disabling conditions from disease or injury.
- Develop and update individualized physical treatment programs and patient Plans of Care (POC).
- Complete and document patient evaluations, tests, and all related therapy documentation per regulatory and departmental standards.
- Provide and progress treatment according to the Plan of Care.
- Coordinate patient care with other healthcare providers and support team members.
- Provide clinical supervision to assistants and aides; serve as clinical instructor for assigned students.
- Maintain compliance with local, state, and federal regulations, including patient privacy and safety.
- Participate in quality improvement processes and interdisciplinary projects.
- Maintain a clean and safe working environment, including proper equipment use and disinfection.
- Perform general clerical tasks as needed, such as updating patient appointments and managing electronic health records (EHR).
Required Qualifications
- Valid Physical Therapist license in California
- Basic Life Support (BLS) Certification
Schedule
- Day shift, 5 days a week (Friday through Tuesday)
- 8-hour shifts (8:00 AM to 4:30 PM)
- 40 hours per week
Background and Compliance Requirements
- 7-year background check required within 6 months of start, including social security trace, county searches, sex offender search, and education verification.
- FACIS Level 3 screening required within 2 weeks of start and monthly thereafter.
- EPLS, HHS/OIG, OFAC, and Medi-Cal Suspended and Ineligible Provider List checks required as applicable.
- Education verification of highest level achieved required (not required if highest education is high school).
- TB screening questionnaire required within 90 days of start and annually thereafter.
- Physical exam required within 1 year of start.
- Fit mask test required within 1 year of start and annually thereafter.
- Immunizations and titers required:
- Tetanus/Diphtheria/Tdap (within 10 years)
- Measles (Rubeola), Mumps, Rubella (MMR)
- Hepatitis B series or declination
- Varicella
- Seasonal flu vaccination during flu season (Nov 1 - Apr 30) or signed declination
- Negative TB test or chest x-ray within 90 days of start, renewed annually.
- Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids per CA law).
Additional Requirements
- Must complete and sign facility-specific documents prior to start, including standards of business conduct, vendor training, code of conduct, and regulatory orientation.
- Must provide full 7 years of work history with no gaps in employment longer than 30 days.
- References: 2 professional references required within 3 years for clinical and allied health candidates.
- Unit-specific skills checklist required and must be renewed annually.
