Job Opportunity: Registered Nurse (RN) – Long Term Care (LTC)
Location: Kankakee, Illinois
Contract Length: 13 weeks
Shift: Days
Hours per Day: 12
Hours per Week: 36
Facility & Unit Details
- Facility type: Skilled Nursing Facility (SNF) and Long Term Care (LTC)
- Census: Typically 80-95 residents
- Room setup: 20 rooms are not private
- Electronic Medical Record (EMR) system: PCC
- Patient Ratios:
- RN: LTC 1:25, SNF 1:13
- LPN: LTC 1:25, SNF 1:13
- CNA: 1:10-15 (aim to keep around 10)
Patient Population & Common Diagnoses
- Common diagnoses include feeding tubes, IV antibiotics, post-surgical care, rehabilitation, and long-term care needs
- Respiratory support includes AVAP/CPAP/BIPAP, mostly at night; some patients on continuous support
Job Duties & Responsibilities
- Administer IV antibiotics and fluids (no titratable drips)
- Perform patient assessments, medication administration, dressing changes, and wound vac care
- Collaborate with respiratory therapists (RT) who are on-site during weekdays
- CNAs handle vitals and activities of daily living (ADLs), but not blood sugars
- Admissions and discharges are completed by RNs
- Participation in interdisciplinary team rounds and meetings is encouraged (includes PT/OT/SLP, RT, social workers, dieticians, wound care nurses)
Scheduling & Work Environment
- Shift hours: 5:00 AM to 5:00 PM
- Weekend and holiday work: Every other weekend and holiday required
- No call or standby requirements
- Floating between units possible but generally kept on the same unit during a stretch
- Scrub color: Any professional color accepted
- Orientation: One 12-hour shift on the floor; additional support available if needed
- COVID-19 precautions: Use of PAPR and N95 masks; COVID patients intermixed with others
Required Qualifications
- Valid RN license in Illinois
- Minimum of 1 year of Long Term Care nursing experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Basic Life Support (BLS) certification
- Valid driver’s license
Additional Information
- Pharmacy available on-site 24/7 with Omnicell and medication carts for daily use
- Physician coverage includes hospitalists after hours and nurse practitioners during weekdays
- Interdisciplinary team includes case managers, social workers, and dieticians on-site
