Description
Physical Therapist - Outpatient
Location: Nantucket, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 12 weeks
Job Details
- Outpatient setting only
- Work schedule: 4 shifts per week, each 10 hours long
- Shift start time between 8:00 AM and 8:30 AM
- Shared housing provided at no cost
Required Qualifications
- Valid Physical Therapist license in Massachusetts
- Minimum of 1 year of Physical Therapist experience, including experience within the last year
- Basic Life Support (BLS) certification issued by the American Heart Association or Red Cross
Onboarding and Compliance Requirements
- Completion of FEMA ICS 100 Training (mandatory, no exceptions)
- Fit Test for N95 respirator within 365 days prior to start, updated annually (no exemptions allowed)
- Respirator Medical Clearance Attestation (if fit test required)
- Documentation of MMR vaccination or immunity proof
- TB test documentation within 3 months of start date (skin test or blood test), updated if break >6 months
- Health Statement signed by MD/DO/NP/PA within 365 days of start, updated if break >6 months
- Criminal background check within 45 days prior to start, including international residence/travel history for last 7 years
- Massachusetts CORI background check within 30 days prior to start
- National Sex Offender Registry check within 30 days prior to start, updated annually
- Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days prior to start, updated annually
- Education verification of highest degree achieved
- Social Security Number trace for last 7 years
- Skills checklist completed within 1 year of submission
- Two professional references within 4 years, including at least one supervisor-level reference
- Resume including past 7 years of work experience with city and state
- Confirmation of Assignment/POI form upon acceptance and for extensions
Additional Notes
- Conditional licenses are not accepted
- Fit testing cannot be performed onsite; must be completed prior to start
- Shared housing requires signing a housing agreement, updated if assignment break exceeds 35 days
- Positive TB test requires documentation of a negative chest x-ray (optional but recommended)
- Name change documentation required if legal name differs from credential or profile records

- 8:00 AM to 4:30 PM (4 shifts per week, 10 hours each)
