Registered Nurse - Hospice

Location: Basking Ridge, New Jersey
Employment Type: Travel Clinical Nursing
Contract Length: 21 weeks

Job Description

• Perform patient assessments to determine appropriateness for hospice care, learning needs, and progress toward goals.
• Develop, implement, and evaluate individualized plans of care for patients and families based on evidence-based knowledge.
• Maintain visit productivity standards and demonstrate knowledge of hospice regulations and insurance reimbursement guidelines.
• Complete admission, discharge, and re-visit assessments and documentation within required timeframes.
• Use age-appropriate teaching methods and document patient education tools used.
• Prioritize patient care needs and communicate progress with the interdisciplinary team (IDT). Participate in IDT meetings every two weeks.
• Perform Home Health Aide (HHA) supervisions according to standards.
• Maintain a safe environment by following infection control measures and reporting infections.
• Demonstrate knowledge of pharmacology and safe medication management, including routine medication reconciliation and maintaining current medication orders.
• Participate in performance improvement activities, including reviewing health records and attending utilization review meetings.
• Provide orientation, mentorship, and cross-training to new employees and colleagues.
• Assist with agency growth by accepting additional assignments as requested.

Required Qualifications

• Valid Registered Nurse (RN) license in New Jersey.
• Minimum 2 years of hospice nursing experience, including experience within the last year.
• Basic Life Support (BLS) certification.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing.
• Valid driver's license.

Preferred Qualifications

• Previous ICU experience.
• Associate's Degree in Nursing (ASN) or Diploma with RN license in New Jersey.

Shift & Hours

• Shift: AM (11:00 AM – 7:00 PM)
• Hours per day: 7
• Hours per week: 37.5

Additional Information

• Work every other weekend (EOW).
• General Inpatient (GIP) experience preferred.
