Description
Physical Therapist (Outpatient/Inpatient) - Travel/Contract
Location: Mullins, South Carolina
Contract Length: 13 weeks
Position Details
- Specialty: Therapy/Rehabilitation (Physical Therapist)
- Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM – 4:30 PM)
- Weekend Requirement: No
- On-call Requirement: No
- Holiday Requirement: Possibly
- Floating: May be required to float within scope to meet facility needs or between like units/sister facilities
- Facility Type: Hospital-based inpatient and outpatient rehab unit
- Charting System: Epic

Required Qualifications
- Valid Physical Therapist license in South Carolina
- Minimum 1 year of Physical Therapist experience within the last year
- Completion of at least one travel assignment as a Physical Therapist
- Basic Life Support (BLS) certification
Compliance & Documentation Requirements
- Background Check: Annual, including 7 years of residence/work history, national and state criminal checks, sex offender registry, FACIS 3, and international searches if applicable
- Employment Verification: Last 3 clinical employers verified (exceptions apply)
- References: Proof of 2 clinical references or hospital evaluations
- Performance Evaluation: Must be dated within 12 months of start
- Skills Checklist: Must be updated annually for applicable units
- Physical Exam: Initial and updated annually, attesting ability to meet physical demands
- Drug Testing: 10-panel drug screen within 90 days of start (no instant tests)
- Immunizations & Titers:
- Tetanus/Diphtheria/Tdap within 10 years or declination form required
- Measles (Rubeola), Mumps, Rubella, Varicella: Positive titer or 2 vaccines; low titer requires boosters
- Hepatitis B: Titer, vaccine series, or declination required
- TB Test: Negative 2-step PPD or Quantiferon Gold within 90 days; if positive, chest X-ray proof required
- Annual TB Questionnaire required
- COVID-19 Vaccination: Not required but highly preferred; vaccination records must be uploaded or declination form completed
- Flu Vaccine: Required during flu season (Aug 1 – Mar 31); medical/religious exemptions accepted but no declinations; documentation must be uploaded by December 5 deadline
Facility-Specific Requirements
- Photo: Passport-style photo with solid light background, frontal view, shoulders up
- Orientation: Completion of assigned modules prior to start; failure to complete results in being sent home
- Signed Documents: Assignment acknowledgment, code of conduct, employee confidentiality agreement, systems user agreement, HR policy manual, job description, HIPAA confidentiality agreement, employee info, and other facility-specific forms
- Radiation Badge: Required if assigned to OR, Cath Lab, EP Lab, or Radiology; not required for MRI, Mammography, or Ultrasound techs
- Safety & Competency Exams: Medication/pharmacology competency and other required competency exams as applicable

- Guaranteed Hours: Facility may call off 1 shift per 2 weeks
- Hurricane Season (June-November): Employees and contractors may be assigned to emergency response teams; new or extension assignments during peak months (Aug-Sept) should be considered carefully
