Required Qualifications

• Valid RN license in Ohio
• Cardiac Stepdown experience within the last year
• Associate of Science in Nursing / Associate Degree in Nursing
• Driver's License

Onboarding / Compliance Requirements

• 2-step TB test or IGRA with negative results within the last 6 months
• Influenza vaccine documentation required from October through March
• If declining flu vaccination, completion of the required masking and influenza declination forms and mask use while working
• If requesting a medical exemption, completion of the required medical accommodation form plus a physician-signed letter on medical office letterhead
• Background check completed within 6 months of start
• OIG search completed within 6 months of start
• SAM and OFAC screening completed within 6 months of start
• Drug screen completed within 60 days of start
• Safety and compliance training modules completed before clearance
• Badge photo upload

Notes

• If the TB test is positive, a clear chest x-ray and current TB questionnaire are required
• If there is a break in assignment greater than 60 days, a new drug screen is required
• If there is a break in assignment of 6 months or more, a new background check is required
