Physical Therapist - Acute Care Hospital (Travel/Contract)

Location: Pleasant Prairie, Wisconsin
Contract Length: 13 weeks
Shift: Weekend Days, 4 shifts of 10 hours each (7:30 AM - 6:00 PM)
Hours per Week: 40

Job Description

• Work in an acute care hospital setting.
• Weekend shifts only.
• Locals accepted, but at a lower rate.
• First-time travelers accepted.

Required Qualifications

• Valid Physical Therapist license in Wisconsin.
• Minimum 1 year of Physical Therapist experience within the last year.

Certifications & Competencies (as applicable)

• BLS (Basic Life Support) – ARC or AHA accepted.
• ACLS (Advanced Cardiac Life Support) – ARC or AHA accepted (not required for CNAs).
• PALS (Pediatric Advanced Life Support) – ARC or AHA accepted.
• NRP (Neonatal Resuscitation Procedure) – as required.
• NIH Stroke Certification – required for ICU, IMC/IMCU, inpatient, Med Surg, Step Down RN positions (may not apply to PT but check facility requirements).
• Medication Test/Pharmacology Competency Exam – required as applicable.
• Color Blind Testing – required for dialysis/kidney center staff only.
• Competency Exam – required as applicable to profession/specialty.

Background & Screening Requirements

• 7-year background check within 1 year of start, including SS trace, county searches for all locations lived/worked/schooled, nationwide search, and sex offender registry.
• Wisconsin Caregiver Search Results required within 90 days of start (agency to run).

Health & Immunization Requirements

• Physical exam within 1 year of start.
• Negative QGold TB test within 1 year of start (TSpot/PPD not accepted). Negative chest X-ray within 60 days if applicable.
• Immunizations or positive titers required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Varicella
  • Hepatitis B (titer, series, or declination)
• Seasonal flu vaccination or approved medical/religious exemption required during flu season (October to March).
• Negative 10-panel drug screen with expanded opiates within 14 days before start (instant tests not accepted). Positive marijuana test is not disqualifying.

Documentation & Other Requirements

• Photo for badge (passport-style, shoulders up, no filters).
• Confidentiality statement signed prior to start.
• Compliance checklist to be completed by agency representative.
• References: 2 professional references verified within 2 years of start. For inpatient RNs only, one must be from most recent acute care assignment (not applicable to PT).
• Annual skills checklist as per agency standards.

• Prior travel experience is not required.
• Locals accepted at a lower rate.
