Physical Therapist - Home Health (Travel/Contract)
Location: Roseburg, Oregon
Contract Length: 13 weeks
Job Overview
- Provide physical therapy services in a home health setting with home visits and periodic office meetings.
- Average patient load: 5-6 patients per day, with 5-10 patients assigned daily equating to roughly 6-8 points per day (30 points expected weekly).
- Types of cases: Start of Care and Follow-Ups.
- Schedule: Primarily Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours). Some positions may have a 4x10 schedule (Friday-Monday). Weekend and on-call requirements vary and should be discussed during the interview.
- Must be willing to travel up to 75 miles from the home clinic, averaging about 100 miles per week. Mileage reimbursed at IRS standard rate (patient-to-patient travel compensated; commute to/from first and last patient is not billable).
- Local candidates accepted at the same rate.
Required Qualifications
- Valid Physical Therapist license in Oregon
- Physical Therapist (1 year of experience)
- Physical Therapist experience within the last year
- One completed travel assignment
- BLS issued by American Heart Association or American Red Cross
- Experience in a Home Health setting
- Driver's License
- Agency-required liability insurance.
- Experience with PointCare and Homecare Homebase (HCHB) electronic medical record systems is required.
- Must be willing to comply with all required background checks, health screenings, and facility-specific documentation prior to start.
Productivity Expectations
- Weekly productivity units: 30-32 points.
- No guaranteed hours; if patient census is low, floating to a nearby clinic may be discussed to maintain hours.
Compliance and Documentation Requirements
- Must complete and sign various compliance documents including confidentiality forms, privacy policies, HIPAA, code of conduct, and COVID-19 policy attestation.
- Required background checks include:
- 7-year standard background check with SS trace, county, nationwide, sex offender, and state-specific searches.
- EPLS/GSA/SAM and HHS/OIG checks within 14 days of start, updated monthly.
- Fingerprinting as required by state regulations (varies by state).
- Education verification for degree applicable to profession.
- Health requirements include:
- Physical exam within 12 months confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months (no pending results at start).
- Proof of immunizations or titers for Measles, Mumps, Rubella, Varicella, Hepatitis B.
- 10-panel drug screen.
- Seasonal flu vaccination or documented declination.
- COVID-19 vaccination series or declination.
- Must provide proof of personal or auto insurance valid for home health visits.

- Work history verification: Must verify at least two most recent employers or last 7 years if only one employer.
- On-call and weekend requirements vary by location and should be clarified during the interview.
- Part-time or weekend-only schedules may be considered.
