Job Overview

Specialty: Therapy/Rehabilitation
Location: Crestview Hills, Kentucky
Employment type: Travel/Contract
Setting: Home health with home visits and periodic office meetings
Shift: Day, 5x8 hours, 8:00 AM–5:00 PM
Contract length: 13 weeks

Responsibilities

Provide home health physical therapy services
Manage start-of-care and follow-up visits
Travel between patient homes within the assigned territory

Required Qualifications

Valid Physical Therapist license in Kentucky
Physical Therapist (1 year of experience)
Physical Therapist experience within the last year
One completed travel assignment
BLS issued by American Heart Association or American Red Cross
Experience working with Adult patients
Experience in a Home Health setting
Driver's License

Schedule Notes

Weekend coverage varies by location and profession
On-call requirements vary by job and location
Most assignments use a standard Monday–Friday schedule

Additional Details

Average patient volume: 5–6 per day
Productivity expectation: 30 points per week
Local candidates are accepted
