Description

Home Health/Hospice RN

Location

Iron Mountain, Michigan

Contract

13 weeks
40 hours per week
Day shift, 8 hours per day

Required Qualifications

Registered Nurse license
BLS certification
Recent home health experience within the last 2 years
OASIS documentation proficiency
Wound care experience
Excellent infection control practices in the home environment

Schedule

Monday through Friday, 0800–1630
Rotating weekends
Rotating holidays
Evening on-call rotation
One weekend on call per month
One evening on call per week

Patient Population / Case Mix

Home health and hospice patients
Common needs include CHF, diabetes, renal disease, wound care, IV therapy, and management of drains and other devices

Responsibilities

Provide home health and hospice care
Complete SOCs, recerts, and resumptions
Participate in on-call coverage

Additional Notes

All clinicians must clock in and out at the facility
Candidates living within 80 miles of the facility are considered local
If previously worked there, the clinician must have an end date at least 6 months before submission
