Required Qualifications
- Valid Physical Therapist license in California
- Physical Therapist (1 year of experience)
- Physical Therapist experience within the last year
- BLS - Basic Life Support issued by American Heart Association
- Experience in a Home Health setting
- Driver's License
- Experience with OASIS

- Valid passport copy before start
- I-9 attestation

- EPLS/GSA/SAM check
- HHS/OIG check
- Personal or auto insurance
- Auto registration

- Physical exam within 12 months of start
- Tdap within the past 10 years
- Negative TB test or chest X-ray within 1 year of start
- Measles, mumps, and rubella immunity documentation required for candidates born in 1957 or later
- Hepatitis B titer or vaccine series
- Varicella immunity documentation
- Negative 9-panel drug screen within 1 year of start
- Seasonal flu documentation required year-round
- COVID vaccination or approved medical/religious exemption required before start

- Guaranteed hours are based on completed visits: 25 visits equals 40 hours
- Mileage between patients is reimbursed at IRS standard
