RN Care Manager
Location
Chapel Hill, North Carolina
Schedule
- Days
- 36 hours per week
- 13-week contract
Required Qualifications
- Valid RN license in North Carolina
- Associate of Science in Nursing / Associate Degree in Nursing
- Case Management (2 years of experience)
- Case Management experience within the last year
- Experience in an Acute Care setting
- Acute Hospital Case Management Experience
- Care Manager experience
Role Overview
- Provides ongoing patient assessment, planning, implementation, and evaluation to support care coordination, utilization review, and discharge planning
- Coordinates with the multidisciplinary team on discharge readiness, barriers to discharge, and transition planning
- Participates in care management meetings and follows up on action items
- Educates patients, families, and care teams on community resources and post-discharge support
- Documents assessment and plan of care in Epic
