PT Home Health
Job Overview
- Travel/contract physical therapy role in home health
- Day shift, 5x8 schedule
- 40 hours per week
- 13-week assignment
- Home visits with periodic office meetings
- Average patient load: 5–6 patients per day
- Productivity expectation: minimum 30 points per week
- Territory includes Kent County and travel up to 75 miles from the home clinic
Required Qualifications
- Valid Physical Therapist license in Delaware
- Physical Therapist (1 year of experience)
- Physical Therapist experience within the last year
- One completed travel assignment
- BLS issued by American Heart Association or American Red Cross
- Experience working with Adult patients
- Experience in a Home Health setting
- Driver's License
- Experience with Homecare Homebase
Schedule Notes
- No weekend rotation
- No on-call requirement
- Local candidates accepted
Compensation / Travel Notes
- Mileage reimbursed at IRS standard rate
- Travel time is compensated from patient home to patient home
- Commute to the first patient and from the last patient is not billable
Additional Considerations
- EMR used: HCHB
- Start-of-care and follow-up visits are the primary case types
