    Description

    PT Acute Care Hospital

    Role Overview

    • Full-time physical therapist in a hospital setting with a focus on acute care and inpatient rehab.
    • Travel/Contract assignment in Crescent City, California.
    • Day shift: 5x8 hours, 8:00 AM to 4:30 PM.
    • 40 hours per week.
    • Contract length: 13 weeks.

    Required Qualifications

    • Valid Physical Therapist license in California
    • Physical Therapist (2 years of experience)
    • Physical Therapist experience within the last year
    • BLS - Basic Life Support issued by American Heart Association or American Red Cross

    Background / Compliance

    • 7-year background check required within 6 months of start.
    • Sex Offender Search required within 90 days of start.
    • Education verification required for highest level achieved, unless highest education level is high school.
    • FACIS Level 3 search required within 2 weeks of start, then monthly after start.
    • California Medi-Cal Search Result required if FACIS III cannot be run monthly.
    • EPLS/GSA/SAM required annually.
    • HHS/OIG required annually.
    • TB screening questionnaire required within 90 days of start and annually.
    • Negative TB test or chest x-ray required within 90 days of start.
    • Physical exam required within 1 year of start.
    • Fit mask test required within 1 year of start and annually.
    • Negative 9-panel drug screen required within 6 months of start.
    • Color blind exam required if applicable and must be in person.
    • Employment application must include full 7 years of work history with no gaps.
    • Work history verification required for up to 7 years of employment history, with gaps over 30 days explained.
    • Unit-specific skills checklist required.
    • References are an Aya requirement only; Sutter does not require references.

    Certifications / Training / Documents

    • Candidate must review and sign the job description, standards of business conduct, general regulatory orientation, vendor training, code of conduct, and overpayment refund policy prior to start.
    • TB questionnaire must be signed by both the candidate and a healthcare provider.
    • Flu vaccination documentation required during flu season unless exempted by signed declination.
    • Tdap documentation required.
    • Measles, mumps, rubella, hepatitis B, and varicella immunization documentation required.
    • RespSafety clearance required prior to start.
    • State license verification required within 30 days of start.
    • NURSYS verification required within 30 days of start.
    • Tech Cert Verification - National required within 30 days of start, as applicable.
    • Medication test/pharmacology competency exam required as applicable by profession.
    • Competency exam required as applicable to profession.
    • Sutter Health - Epic Questionnaire required for RNs only, so not applicable here.

    Duties

    • Assess, plan, organize, and participate in rehabilitative programs to improve mobility, relieve pain, increase strength, and correct disabling conditions.
    • Complete patient evaluations, tests, documentation, and plan of care updates.
    • Provide treatment and progress patients according to the plan of care.
    • Coordinate care with other providers and support team members.
    • Provide clinical supervision to assistants and aides.
    • Serve as clinical instructor for assigned students.
    • Maintain safety, infection control, privacy, and regulatory compliance.
